Register a Child
All information must be completely filled out in order for your child to be registered for VBS. If there is a question that does not pertain to your child, simply write the word NONE in the blank, this way you will be able to proceed. All questions require an answer.
CHILD PICK - UP - Please remember all children are to be released at 1:00 pm. We do not provide any babysitting services or child care. You are responsible to be on time.
Please remember to LIST EVERYONE who has the right to pick up your children from VBS. Your child/children will only be released to those authorized. If they are not on the list, your child/children will NOT be released. Please tell the person picking up your child/children that we may ask for IDENTIFICATION. Do not be offended, we want everyone's children making it home safely at the end of the day. You must come inside to pick up your child/children. No child/children will be released from the building to run out to a car with adults waiting.
PHOTO RELEASE - This gives us permission to post pictures on NorthLife Fellowship Baptist Church's webpage, Facebook page and possibly Fort McMurray Today newspaper. It does not give us permission to sell, publish or use your child's photo for any type of promotional materials for a current or future program. It is a standard form used by all organizations to make sure that your child's picture can be taken and used in the final video at the celebration service, and again posted on the above pages. If you choose to say NO, and your child's picture cannot be taken, they will receive a blue sticker on their name tag. That means we will pull them from any group photos that are taken, and they will not be allowed to be in the final video shown at the celebration service, or be able to participate in the service because it is streamed on Sunday Mornings.
When you get to the question that asks for the State, Zip Code. In the Pull Down Menu for that question, please scroll down to Alberta, and then you will be able to put in the correct postal code.
Tide Pool Preschool - Parents please remember in order for your child to attend our Preschool program, your child must be born in 2018 or 2019. Age 4 and 5 years old. No one under the age of 4 is being accepted. Thank you.
