By completing this form I hereby give my permission for my child to attend the activities sponsored by NEW HARVEST ASSEMBLY OF GOD 3864 US HWY 127 S FRANKFORT, KY 40601, 502-607-0306, I do also release NEW HARVEST ASSEMBLY OF GOD, staff member(s), and sponsor(s)/volunteers(s) from liability for accidents or injuries incurred during these church sponsored activities. In addition, I do give my permission for the staff member(s) or sponsor(s)/ volunteer(s) of the church sponsored activities to seek, administer, and authorize medical attention and care should this be deemed necessary.*
